Appointments
You time is valuable and we are committed to an office schedule that values your time and runs smoothly. Please arrive 20 minute early if you are a new patient or 5 minutes early if you are an established patient to take care of any administrative or financial details prior to your appointment. Emergencies are common in Urology, and your appointment may occasionally be delayed or rescheduled. If you cannot keep your scheduled appointment, please notify us as soon as possible to avoid a possible charge.
Keep in mind that a urine sample may be needed at every appointment. You may request a sample cup on arrival to minimize any discomfort.
YOUR INSURANCE CARD AND PHOTO I.D. WILL BE REQUIRED AT EVERY VISIT FOR NEW OR ESTABLISHED PATIENTS.
Prescription Refills
We will call in prescription refills for established patients who have been seen in the last year in our office. Please allow 1-2 days for this to occur. A urine examination may be required prior to some refills.
Fees and Payments
We expect payment at the time of service. We believe you will find our fees competitive for our area. We can assist you with arranging credit or payment plans, and will only bill you for your balance.
If you are Medicare eligible, we will file your claim for you. Your reimbursement will come with an "Explanation of Benefits" (EOB). This EOB is necessary to file with any secondary insurance.
You are responsible for complying with the requirements of your insurance plan. If referral is needed to see a specialist, you are responsible for obtaining that referral from your primary doctor. If you fail to notify us of necessary referral, your plan may not cover your care, and you could be faced with an unexpected large debt.
Necessary care will not be denied for financial reasons. Special payment arrangements can be made.
Questions
Do not hesitate to ask any questions about your diagnosis, recommended treatment, studies or financial aspects of your care. We want you to be informed regarding all aspects of your care.
